Hey Bob!
That is a Ron Francis HL-03 which is the same as a 1960s/1970s era General Motors headlight switch. You could walk into a NAPA store and tell them that you want one for a 1970 Chevy pickup, Impala, whatever because they're all the same.....but I bet when edwardb sees this he'll make it even easier and reply with the actual part number and recommend an online source

ACDelco D1588 is also the same thing. I've found the branded ACDelco part to be a bit better quality than the kit supplied switch. Could be available at NAPA, but haven't tried. Little over $20 on Amazon for the genuine article.
