looking at the manual it says to bolt the sidepipe brackets through the 2x2 square tube and use lock nuts on the end of the bolt in the foot boxes. do people hit their heels on the bolt ends? do you carpet over these? is there an acceptable alternative like rivnuts in the 2z2 instead of a through bolt?

No sure but I think the bolts run parallel to the ground and floor. This puts them through the 2x2 tube from front to rear not up & down.
But then I'm just guessing.

You drill the hole then slide the bolt through pointing back toward the seat pan. The nut and rubber grommet are under the car. The only part in the footbox is the head. I carpeted over it and never thought about it again.

I copied Edwardb and used 5/16 nutserts rather than drilling all the way through the 2" tubes. You can read about it in his build thread here. Turned out extremely solid and nice not to have the bolt heads in the footwell.

Railroad's design is really nice for adjustability; my approach is adjustable but with washers in the vertical direction. I put two Rivnuts into the bottom surface of the 2 x 2 tube and used a piece of 2x2 angle iron with a slot machined into mount the FFR hardware. The slot gives you in/out, washers under the angle iron give you up and down.

Lot of interesting approaches listed here. I kept it simple and drilled through both sides of the 2x2. Unless you put your head in the foot box, you can't see the bolt head. Because the bolt head is smooth, there is really no risk of injury. If you decide to use floor mats they cover everything up and it is a non-issue.
